Neuromodulators: Botox, Daxxify, Xeomin & More

Botox, Dysport, Xeomin, Jeuveau, Daxxify and Letybo all relax the same muscles, but they are not interchangeable. Here is how they differ and how I choose between them.

Walk into any med spa and you will hear six brand names thrown at you as though they are interchangeable. They are not. They all relax the same muscles, but they differ in how fast they work, how long they last, how far they spread, and what is actually in the vial. Here is what separates them, and how I decide which one belongs in your face.

What Neuromodulators Are, and How They Work

A neuromodulator is an FDA-approved injectable that smooths expression lines by relaxing the muscles beneath your skin. When you frown, squint, or raise your eyebrows repeatedly over years, the skin above those muscles creases. A neuromodulator gently interrupts the signal between nerve and muscle, so those repeated contractions ease up. The result is smoother skin without the frozen look, as long as it is placed with care.

Think of it this way. You are not paralyzing your face. You are giving overworked muscles a break so that new collagen can form, and you can still make expressions.

The Main Neuromodulators, and What Sets Them Apart

There are five FDA-approved options I work with here in Loomis, and a sixth that is becoming more widely available. Each one has its own profile.

Six neuromodulators, one mechanism. The differences are in formulation, spread, onset and duration, which is exactly what the consultation is for.
  • Botox is the original and the most recognizable name. It has been in use the longest, so there are decades of clinical data behind it. Many people choose it simply because they know what to expect.
  • Dysport works similarly to Botox but tends to spread a little more once injected, which can suit larger areas like the forehead. It can also show results slightly faster for some people.
  • Xeomin is sometimes called naked Botox, because it is the same active ingredient without the extra proteins around it. A solid choice if you want something streamlined.
  • Jeuveau is the newest of the traditional options, another clean formulation without unnecessary additives. It works the same way as the others.
  • Daxxify is in a class of its own. It is the only long-lasting neuromodulator designed to smooth frown lines, and it is peptide-powered rather than relying on the same mechanism as the others. It is made in California and is free from human blood products and animal byproducts. Because it typically lasts around six months or more, it is worth considering if you want fewer appointments.
  • Letybo is another option in the family, though availability varies by location.

Which One Is Right for You?

There is no single best neuromodulator. The right choice depends on your goals, your budget, how your skin responds, and what you value most.

If you want proven reliability and the longest track record, Botox is an excellent choice. If you are interested in a longer-lasting option and willing to invest more upfront, Daxxify may appeal to you. Some people prefer Xeomin or Jeuveau simply because they like the idea of a cleaner formulation. Others find that one brand works better for them personally than another, and that is completely normal.

Worth Knowing

Your skin is individual. What works beautifully for your friend may feel different on your face. That is exactly why the consultation matters more than the brand name on the box.

Beyond Smoothing Lines: What They Actually Treat

Neuromodulators are not just for forehead lines. They work well on frown lines between the brows, crow's feet at the corners of the eyes, bunny lines on the bridge of the nose, and lines around the mouth. Some people use them preventatively in their thirties or forties to keep lines from deepening. Others wait until lines are more established and want to soften them. Both approaches are valid.

Placed well, they can also do subtle lifting. A lip flip uses a small amount to relax the muscle that pulls the upper lip down, making the lip appear fuller. A softened chin dimple comes from relaxing the mentalis muscle. These refinements require a real knowledge of facial anatomy and a genuine desire to enhance what you have rather than change who you are.

What the Appointment Is Actually Like

When you come in for your first neuromodulator appointment, expect a real conversation. I will ask about your concerns, show you how different areas of your face move, talk through what natural-looking results actually look like, and answer your questions honestly. If you are nervous, I will tell you exactly what to expect. If you are wondering whether you even need treatment, I will tell you that too.

The injection itself takes minutes. There is a small pinch, but it is brief. Most people find it far less uncomfortable than they had imagined.

  1. Return to normal activity right away, there is no downtime
  2. Avoid rubbing the treated area, and stay upright for a few hours
  3. Skip strenuous exercise for 24 hours so the product settles where it is meant to

You might notice a tiny red dot where the needle went in, but it fades quickly. Those small steps are all the aftercare most people need.

Where This Fits in a Bigger Plan

A neuromodulator softens movement. It does not change skin quality, and it will not do anything for texture, tone, or laxity. If lines are etched into the skin even when your face is at rest, movement is only part of the story, and the skin itself needs attention too. That is where NeoGen PSR or microchanneling come in, and why I often talk about them in the same conversation.

Honest answer: if you only ever do one thing, daily sun protection will do more for your face over ten years than any injectable. The injectables work better on well cared for skin anyway.

My Approach

I would rather undertreat and bring you back in two weeks than overtreat and ask you to wait three months for it to wear off. You can always add. You cannot subtract.

Frequently Asked Questions

Most people notice the effect starting at three to five days, with full settled results around 14 days. I usually ask you to wait the full two weeks before judging the outcome, because the product is still taking hold before then.

For most of the traditional products, three to four months is typical. Daxxify generally lasts longer, often around six months or more. Your metabolism, your muscle strength, and how much you exercise all influence it.

Not if it is placed with care. A frozen look comes from treating too many muscles too heavily, not from the product itself. I would rather start conservatively and add at your two-week check than overshoot.

There is a brief pinch at each injection point and that is it. The whole appointment takes minutes, and most people tell me afterward that they had built it up far more in their head than it deserved.

None. You can go straight back to your day. Avoid rubbing the area, stay upright for a few hours, and skip hard exercise for 24 hours so the product settles where it was placed.

Yes. Some people respond better to one than another, and there is nothing wrong with trying a different product at your next appointment. Tell me how the last round felt and how long it lasted, and that guides the next choice.

It depends on what your face is doing, not on your age. If lines are already visible when your face is at rest, treating movement now makes sense. If they only appear when you move, you have more time than you think.

A neuromodulator relaxes muscle, so it softens lines caused by movement. Filler adds volume, so it restores structure that has been lost. They solve different problems, and plenty of plans use both. You can read more on my dermal fillers page.

Yes. A neuromodulator placed into the affected area can interrupt the signal that tells overactive sweat glands to produce sweat. It is a separate conversation from cosmetic treatment, so bring it up at your consultation and we will assess it properly.
